Every person is unique, possessing a sense of what is "right" for him or her. Each individual also has values and beliefs that guide his or her decisions about health, finances and other personal matters. Each person has the right to make his or her own choices, even if others do not agree with them.
To make sure that your wishes are known and carried out throughout your whole life, you must have a way of letting others know what you want if you become unable to tell them yourself. It is very important to state your desires NOW, in writing, while you can clearly express them. This process of documenting your wishes is often referred to as "Personal Planning."
The Virginia Department for the Aging (VDA) has materials which can help guide you through this process, as follows:
- Advance Medical Directive (Living Will) Form
- Power of Attorney Materials
- Retirement & Finances Book
- Tools for Life Planning in Virginia Book
You can download a copy of each of these documents by visiting our Publications pages and looking under the subject headings "Advance Medical Directive (Living Will)," or "Legal Issues & Services". You may also find the information on our Advance Medical Directive web page helpful. If you need more information or assistance, please contact us.
